Physical Forms

Interscholastic Athletic Program:

Students in grades 6-8 are offered a wide variety of opportunities to participate in interscholastic athletic competitions against neighboring schools. Practices and games are held immediately after school four to five days a week and do not meet in the evenings or on weekends. The school district provides uniforms and transportation to and from the events. Game schedules, directions, and weather related announcements can be found in the athletics section of the school website.

State law requires that all candidates for a school athletic squad have a medical examination within 365 days prior to the first practice session. In addition to the medical form which must be filled out by a medical doctor, a permission form is required for each season. These forms are available on the school website or can be picked in the school health office. It is highly recommended to complete the annual physical examination in the summer so the physical is valid for the entire school year. These forms are then reviewed by the school doctor. Students will not be allowed to participate on a team unless these forms have been turned in to the school nurse prior to the school doctor’s scheduled appointment at the school.   This is especially crucial in the fall as the school doctor is scheduled to review the forms during the first week of school. All students participating in a fall sport must turn their forms into the health office at the scheduled time in August.

For the sports that require try-outs, students and parents will sign off on an additional form outlining try-out guidelines and procedures. These forms will be given out by the coach at the beginning of the season or may be downloaded from the athletics section of the school website.

Please note the following school policies which relate to athletics:

Academic Eligibility

  • During the first trimester period of each academic year, all pupils in grades 6, 7, and 8 will be eligible.
  • Any 6 th , 7 th , or 8 th grade pupil who receives two D’s or an F at the end of the first trimester is ineligible for any co-curricular or athletic activity for the second trimester.
  • Any 6 th , 7 th , or 8 th grade pupil who received two D’s or an F at the end of the second trimester is ineligible for the third trimester.

Participation In Physical Education Class

Students must participate in physical education class in order to practice or play in the after school practice or game.


Students must be in school by 10:30 a.m., for 4 hours of instruction to try-out, practice or play in a school sanctioned event that day.


Students are expected to adhere to the following sportsmanship code:

  • Understand and follow the rules of the contest
  • Recognize skilled performance of others regardless of affiliation
  • Display respect for all individuals participating in the athletic event including; officials, coaches, team-mates, opponents and spectators
  • Treat opponents in an empathic manner
  • Congratulate opponents in victory and defeat
  • Display appropriate behavior at all times

Please be advised of the following procedures regarding medical forms needed in order to participate in the Ho-Ho-Kus School athletics program.

Under state law, the school doctor must verify the accuracy of the completed form and officially validate it. Only original forms will be accepted; the school doctor will not accept photocopied or faxed forms. All forms must be submitted before the school doctor visits the school to review them. Once the school doctor has been at the school and reviewed the forms, there will be no more forms accepted and your child will not be eligible for that season . Therefore, it is critical that the forms are turned in prior to the due date for each season. These dates are posted in Wednesday Notes, announced on daily announcements and in physical education classes. If a form is not filled out completely, it will be given back to you and you may risk missing the school doctor’s visit, thus your child would not be eligible to participate that season.

All athletes must participate in a minimum of 6 practices in order to play in a game; therefore, the school doctor’s visits are scheduled accordingly, well before the first game of the season. A Supplemental Health History Form  must be filled out and submitted for the physician’s review for each season.

Practices take place almost every day immediately after school until 4:30 or 5:00 and students are expected to attend all scheduled practices and games.

The school provides coaches, uniforms, equipment, and transportation to all away events.

There is a $100 participation fee for the first sport per school year, $50 for the second per school year, and there is no charge if the student participates in a third sport. The participation fee is due before the athlete is issued a uniform. If the case of financial hardship, please speak to the school principal.

Prior to competing, parents and students are required to acknowledge receipt of the Ho-Ho-Kus Concussion Management Policy and Sudden Cardiac Death paphlet. These forms will be distributed by the coaches during preseason and must be returned before the start of the season. These forms can also be found at the buttom of this page.
